Structured Templates: Editing 'Assessment & Plan' Section

The Assessment & Plan (A&P) section in Freed’s Structured Templates is fully customizable, allowing you to document clinical reasoning and care plans in a format that works best for your workflow. Whether you prefer a concise problem list or a narrative-based plan, you can tailor this section using the Template Builder.


Problem-Based vs. Separate Problem List

Freed offers two pre-built formats for the A&P section:

  1. Problem-Based Assessment and Plan

    • Each problem includes both an assessment and a plan within the same block.

    • Ideal if you prefer to handle each diagnosis as a single unit.

  2. Separated Problem, Assessment, and Plan

    • Creates 3 separate subsections: Grouped Problem, Grouped Assessment, and Grouped Plan.

    • Ability to customize format of each subsection.

    • Useful for clinicians who want to view each subsection separately, and want fine-tuned control of the formatting of each subsection.

You can switch between these formats within the Template Builder by selecting the caret of the subsection and selecting "Change" and selecting your preferred option.


Adding New Subsections to the A&P

You can expand or modify the A&P section by adding new subsections. In the Template Builder, navigate to the Assessment & Plan section and click “New Subsection” at the bottom. Choose from the following options:

Pre-Made Subsections

Select from a list of common A&P elements created by Freed, such as "Medical Decision Making" or "Risk and Safety Assessment". These are great for quickly adding standardized components to your workflow.

Fixed Text

Insert static text that will appear exactly the same in every note.

Custom Subsection

Design your own AI-powered prompt. This is an advanced feature that lets you instruct Freed on how to generate content using your own wording, tone, and structure.
