Structured Templates allow you to have consistent control over the structure, formatting, and content of your clinical notes by using Freed’s Template Builder.
By building and customizing an outlined structure, Structured Templates allow you to organize the format of your notes before a note is even generated, and give you the ability to use advanced features such as custom subsections.
About Structured Templates
Structured Templates are established note templates created using Freed’s Template Builder. You can choose from a variety of pre-built templates from the Freed Template Library, or you can create your own from scratch.
They are ideal for clinicians or groups who require consistent, repeatable formatting and prefer complete control over how each part of the note is structured.
All users start with a variety of default Structured Templates in their library, determined by the medical specialty selected during the sign-up process.
In the template selector, Structured Templates are indicated by a blue icon (containing rectangles and a lightning bolt).
Because Structured Templates are defined using a pre-built outline, the Learn Format feature is not available when using a Structured Template.
If you prefer that Freed learns how to format your notes based on your edits, or if you need further customization, we recommend using a Learned Template.
When to Use Structured Templates
Structured Templates are a good fit when you:
Need standardized documentation across a team or clinic
Want to pre-define the layout of SOAP notes
Require consistent section order and formatting
Need templates for visit types beyond just New and Returning Patients
Using the Template Builder
You can create a new template from scratch or duplicate a Freed-made template to customize it.
Step 1: Open the Template Builder
To create a new Structured Template:
Click the Templates icon in the left sidebar.
Click “+ New Template” in the upper right-hand corner of the Template Library.
To customize an existing Structured Template made by Freed:
Click the Templates icon in the left sidebar.
Click the name of the template you wish to customize.
Click the Duplicate & Edit button in the bottom right corner.
Step 2: Edit the Template in the Template Builder
You can use the following features in the Template Builder:
Add Subsections
Click the New Subsection dropdown menu to select from pre-made subsections to insert into your note.
These subsections have been developed by Freed and are meant to capture specific details in a predefined way. There is no way to edit the content within these subsections.
Reorder Subsections
Use the handle icon to drag-and-drop subsections to organize them in your preferred order.
Remove Subsections
Expand a subsection and click the red trash icon next to the subsection's title to remove this subsection. This cannot be undone.
Hide or Show the Subsection Heading
Click the “eye” icon to toggle headings on or off.
Style and Length Settings
Expand each subsection block to show options to define the level of detail and the format (paragraph vs. bulleted list) that you'd like.
Add Fixed Text
Add a Fixed Text subsection to insert text to appear verbatim in the note. For example, you can insert a telehealth consent statement or clinic information.
Add a Custom Subsection
You can use this advanced feature to write your own prompts to capture specific information that is not available as a premade subsection.
Please note that using a Custom Subsection is an advanced feature that may produce unexpected results. Due to the technical complexity of prompt writing, Freed cannot guarantee that results generated from a custom prompt will be satisfactory.
If you use this feature, you may need to iterate on your custom prompt until you achieve the desired result. Freed's support options for custom prompts are limited - our team can offer syntax guidance but cannot write prompts on your behalf.
Step 3: Save your Changes
Click the Save button in the bottom right corner to apply your changes to the template.
Step 4: Use your Structured Template
If you'd like to regenerate an existing note to incorporate the updates you've made to your Structured Template, you can follow these steps:
Open the note you wish to regenerate.
Click the Template Selector.
Click on the name of the template you want to use.
If you're already using the desired template, click a different template, let it regenerate, then select the template you wish to use.
