How to use, customize, and manage Structured Templates to create notes that follow a pre-determined format.

Structured Templates allow you to take full control over the structure, formatting, and content of your clinical notes. Built using Freed’s Template Builder, these templates let you define exactly how your SOAP notes should be organized—from section order to custom subsections and formatting style.


About Structured Templates

Structured Templates are manually built note templates created using Freed’s Template Builder. They are ideal for clinicians or groups who require consistent, repeatable formatting and full control over how each part of the note is structured.

You can use Structured Templates for any visit type and can create as many variations as needed.

When to Use Structured Templates

Structured Templates are a good fit when you:

  • Need standardized documentation across a team or clinic

  • Want to pre-define the layout of SOAP notes

  • Require consistent section order and formatting

  • Need templates for visit types beyond just New and Returning Patients

Create a New Structured Template

  1. Open the Template Library before starting a note or while viewing a processed note.

  2. Click “+ New Template” in the upper right-hand corner of the Template Library.

  3. Name your template and begin building each section using the Template Builder.

Using the Template Builder

The Template Builder gives you powerful options to configure the structure of your template:

  • Add Subsections: Include predefined options (e.g., HPI, ROS).

  • Hide or Show Headings: Click the “eye” icon to toggle headings on or off.

  • Reorder Subsections: Use drag-and-drop to organize them in your preferred order.

  • Style and Length Settings: Expand each subsection to control the writing style and level of detail.

  • Free Text Subsections: Insert static text blocks that appear exactly as written in the note.

  • Custom Subsections: Write your own AI prompts to guide content generation.

Customize an Existing Structured Template

  1. Navigate to the Template Library.

  2. Select the ellipsis (...) of the Template you wish to customize and select Edit.

  3. You’ll enter the Template Builder, where you can make edits as needed.

Note: If your note was created using a Structured Template, you will not see the “Learn Format” button. All edits must be made using the Template Builder.


Apply a Structured Template to your Visit

You can apply a Structured Template:

  • At the start of a visit, before selecting “Capture Conversation”

  • After a note is processed, by switching templates and regenerating the note

Once a Structured Template is applied, the entire note will be reformatted according to the template’s design.
