Cause: The Subjective section may not appear as expected if the Learned Template settings are not configured to include it. Feedback on individual notes does not retrain your account or affect future notes.
Solution: Update your Learned Template via Templates in the sidebar to control the presence, structure, and phrasing of sections like Subjective. For one-off fixes, use AI Chat to rewrite or reformat the note without altering your global template.
2. No Note Generated for HPI/Subjective Section
Cause: A very short recording (Freed requires at least 25 seconds of audio) with limited usable audio or frequent pauses can prevent note generation.
Solution: Ensure your recording is longer, continuous, and has clear audio. Avoid frequent pauses during the encounter to capture sufficient data for note generation.
3. Missing SOAP Details or N/A Note
Cause: Missing SOAP details may occur if the transcript lacks content or if the note template fails to populate correctly.

Verify the transcript for the visit to ensure it contains the full details.
Refresh the note by switching to a different template using the template selector in the note view.
Wait for the note to regenerate and check if the SOAP fields populate.
Switch back to your usual template and confirm the note now appears correctly.

Many of these issues — a missing section, an incomplete Assessment & Plan, details that didn't make it in from the transcript — can be fixed directly in AI Chat without changing your account-wide template.
Open the visit note and use the AI Chat panel on the right side of your screen. Describe the problem and what you want fixed, in plain language:
