In Freed, the History of Present Illness lives in the section labelled Subjective. A default Freed note has four sections — Subjective, Objective, Assessment & Plan and Patient Instructions — so anywhere this guide says "HPI", look for Subjective in your note. If you prefer the heading to read "HPI", click the section title in a note and type the name you want; Freed will use that heading on your future notes.

Learned Templates: teach Freed your preferred note structure either by turning on Auto Learn for the template, so it updates itself after qualifying edits, or by clicking Learn format in the visit view to apply your edits on demand. Each learned template keeps one example note showing how your notes should look — open the template and edit that example note to change formatting, level of detail or date style.
AI Chat: Make one-time changes to a specific note by opening the AI Chat and providing instructions (e.g., "Organize HPI by complaint" or "Change HPI to sentences instead of bullet points").
Step-by-Step Guides for Common Customizations
Organizing the HPI Section by Complaint
For a specific note: Use the AI Chat and instruct it to "Organize HPI by complaint."
For all future notes: Use Learned Templates to ensure the HPI is always organized by complaint. Add special instructions like "[Organize HPI by complaint]" in your template.
Changing the HPI Format (e.g., Paragraphs vs. Bullet Points)
To switch to paragraph form: Use Learned Templates to teach Freed your preferred style. (Structured Templates can also enforce paragraph formatting, but they are being sunset and are not available for new accounts.) For a single note, use the AI Chat and instruct it to "Make the HPI section a paragraph."
To switch to bullet points: Similarly, use templates or the AI Chat to adjust the format.
Removing Unwanted Content (e.g., Direct Quotes or Small Talk)
Use Learned Templates to teach Freed to avoid including direct quotes or extraneous information in the HPI section. Add specific instructions like "Avoid using direct quotes from the patient."
For a single note, use the AI Chat to remove unwanted content.
Content missing from a section
Open the visit and use the AI Chat panel. AI Chat can read the note and the transcript and rewrite Subjective, Objective, Assessment & Plan, Patient Instructions, the visit summary, and clinical letters. For example: "Add the risk factors we discussed to the Assessment and Plan."
AI Chat cannot add a brand-new custom section to a note. To change which sections Freed produces, update your template instead.
Including More Details with Dates
Edit the Learned Example for your template to include the desired details and date formatting. Freed will apply these changes to future notes.
Troubleshooting Missing or Incorrect Content

Check the HPI section in your template. If it contains only minimal content, edit and expand it to include the desired level of detail. After updating the template, rebuild the note: open the visit, use the template selector above the note to pick a different template, wait for it to rebuild, then switch back. (Freed does not have a "regenerate" button.)
Greyed-Out or Missing Therapy Sections
Create a new Learned Template or use a prebuilt template from the Template Library to ensure therapy sections populate correctly. Apply the updated template, then rebuild the note by switching templates and switching back — saving changes to a template while the visit is open also offers to rebuild that visit's note.
