EHR push is a powerful button that automatically maps and transfers notes from Freed into AdvancedMD.
How to use the Chrome Extension
Follow the steps below to install and use the Chrome Extension with AdvancedMD.
Important: AdvancedMD launches as pop-up, therefore it is important to launch it as new tab. Workarounds:
For Windows: Click on the top bar, then right-click and click Show as tab.
For Mac: Click Window in the menu bar, then click Show as Tab.

To use EHR Push, install and pin the Freed Chrome extension:
2. Open AdvancedMD as a new browser tab
Launch AdvancedMD, then open it as a browser tab to launch the Freed extension overlay. On Windows: right-click the top bar and choose Show as tab. On Mac: click Window in the menu bar, then click Show as Tab.
3. Record your visit and push
Record your visit, review, and open your patient's chart in AdvancedMD. Click Push to EHR while viewing your patient's note in Freed, and your note will transfer automatically.
How Freed Maps Your Note to AdvancedMD
This section explains which parts of your Freed note land in which AdvancedMD fields, and how to get the most accurate results.
A quick note on terms
This article talks about two different "notes" and two different "templates":

Your AdvancedMD template | The note layout your clinic has configured in AdvancedMD — its tabs and fields (e.g., "New Patient Visit," "Postop Visit"). You already have this; it's not something Freed changes. |
Your AdvancedMD note | The specific patient encounter note currently open in AdvancedMD that Freed pushes into. |
Your Freed template | The structure Freed uses to generate your visit note (which sections it includes, e.g., HPI, Objective, Assessment & Plan). |
Your Freed note | The specific visit note Freed generates from your conversation, using that template. |
The push works by reading your Freed note and placing its content into the matching fields already defined in your AdvancedMD template — automatically detecting whether that template uses tabs or shows everything on one page.
Starting the push
You don't need to be on any particular tab. When you start the push, Freed scans your AdvancedMD note for all its clinical tabs (Subjective/HPI, Objective, Assessment & Plan, Medications, etc.), then visits each one in turn and fills matching fields before moving to the next. The one thing that matters: have the actual AdvancedMD note open — not just the patient's chart — before starting.
Optimizing your Freed note to match your AdvancedMD template
Since your AdvancedMD template's fields are fixed, the best way to improve push accuracy is to make sure your Freed note contains clearly identifiable content for each field you want filled. Field labels on the AdvancedMD side don't need to match your Freed section titles word-for-word — Freed matches by meaning, not exact text.

Chief Complaint / CC / Presenting Problem | A distinct, clearly stated chief complaint, called out separately rather than folded into the first line of HPI. |
HPI / History of Present Illness / Subjective / History | Nothing extra — included by default whenever your note has HPI content. |
Review of Systems (as its own separate field) | Included automatically on newer accounts. On the standard integration, write ROS as part of your HPI narrative (e.g., a "Review of Systems:" block at the end of HPI) rather than expecting a separate field to be filled directly. |
Physical Exam / Exam / Objective / PE / Mental Status Exam / MSE (behavioral health templates) | Nothing extra — included by default whenever your note includes exam/objective findings. Mental Status Exam findings should be written as part of this same Objective/Exam content, the same way physical exam findings are. |
Social History (as its own separate field) | No dedicated mapping today — write social history as part of your HPI narrative. A distinct, separately-labeled Social History field is not currently guaranteed to be filled from a distinct source. |
Separate Assessment field + separate Plan field | Nothing to configure — Freed automatically splits its Assessment & Plan content between the two, keeping each problem's plan tied to its assessment. For example, if your note reads: "Hypertension: BP remains elevated at 148/92 despite current regimen. Plan: Increase lisinopril to 20mg daily; recheck BP in 4 weeks," Freed places the diagnosis heading and assessment narrative in the Assessment field, and the same diagnosis heading with the plan items in the Plan field — so each plan item stays clearly linked to its problem even though it's split across two fields. |
Combined Assessment & Plan field (A&P, Impression & Plan, etc.) | Nothing extra — the full Assessment & Plan content is pushed as one block, per diagnosis. |
Progress Note | Yes, Freed can push here — but only as a fallback, when your template doesn't have a field more specifically labeled "Assessment" or "Assessment & Plan." It's sourced from the same Assessment & Plan content as the row above; "Progress Note" is just another label some AdvancedMD templates use for that same kind of field. |
Clinical Summary | Maps to Assessment content — but only from a block in your note explicitly headed "Clinical Summary." Otherwise it's used as a generic assessment fallback only when no more specific Assessment field exists on the page. |
Care Plan | Common in OB/GYN, fertility, pediatric, and pre/post-op templates. Supported on the newer integration; not a defined target on the standard one. If used, make sure care-plan-style content (follow-up actions, patient education) is clearly identifiable in your note. |
Diagnoses (ICD code field) | Not applicable — Freed never places narrative text here; it's reserved for coded diagnoses. |
Current Medications / Meds / Rx | A standalone medications list with its own header (e.g., "Medications:"), placed inside the History of Present Illness (HPI) section of your note. This does matter: Freed only looks for this list within HPI — the same list placed under Objective or Assessment & Plan won't be picked up by this rule. |
Labs / imaging results (dedicated field) | Inconsistent — sometimes filled from Objective content, sometimes not targeted at all. Keep lab/imaging results within your note's Objective section for the best chance of being picked up. |
Vitals (Height, Weight, and similar single-value fields) | Not filled by Freed. These are discrete numeric-entry fields, and Freed only pushes narrative text — plan to enter vitals manually. |
Patient Instructions / follow-up fields | Nothing extra — included by default whenever your note has a Patient Instructions section. |
Counseling / Patient Counseling | Maps to your note's Patient Instructions content. |
Administrative fields (Emergency Contact, Insurance, Consent, Signature, Visit Type, Date of Service, etc.) | Nothing to do — Freed intentionally skips these no matter what's in your note. |
What Freed deliberately won't do
Won't place clinical narrative into diagnosis/ICD code fields.
Won't fill administrative fields.
Won't guess at missing content — if Chief Complaint or Medications aren't clearly present as their own labeled content in your Freed note, the matching field in AdvancedMD is left blank rather than filled with a guess.
Won't overwrite existing text in your AdvancedMD note — content is added, not replaced.
Some AdvancedMD accounts are on a newer version of this feature that reads your AdvancedMD template directly for even more precise placement — this is rolling out gradually.
