Merging duplicate patient records

Merging duplicate patient records

Freed can combine two duplicate patient records into one, keeping all visits and notes together under a single profile. This is available for Groups plan subscribers with patient sharing enabled — if sharing isn't on, you can still merge duplicates among your own patients.

Note: Merging is only available in the desktop web app. If you're working inside the Chrome extension or an embedded EHR view, you'll see "Patient merging is available in the desktop view."

Where to find it

  1. Open the patient selector (wherever you pick a patient for a visit).

  2. Click the chevron ("View patient profile") next to the patient's name.

  3. In the Patient profile dialog, click Merge with another patient.

How to merge two records

  1. Freed shows a list of likely duplicates (matching name), each labeled Your record or Shared record with date of birth and last-visit info. If the right one isn't listed, use Search for someone else.

  2. Select the duplicate and click Compare records.

  3. Review the side-by-side comparison — Given name, Family name, Date of birth, Phone, Email, Pronouns, EHR link — and choose which record's value to keep for each field, or select Use custom to type a different value. Family name is required.

  4. Read the warning: "Visits from both records will be combined under one patient. This merge cannot be undone."

  5. Click Merge records to confirm. You'll see a "Patients merged" confirmation.

What happens when you merge

  • All visits and notes from both records move under the surviving record.

  • The duplicate record is deactivated and no longer appears in the patient selector.

Important: Merging is permanent — there's no in-app undo, so review the comparison carefully before confirming.

Who can do this

  • Any provider in your group who can already view both patient records can merge them — it isn't restricted to admins.

  • You can only merge patients within the same practice/account; records can't be merged across different subscriptions.

Troubleshooting

If you see "Unable to merge patients. The records may have changed or you may not have access. Go back and review them again," close the dialog, refresh, and try again — the records may have changed since you opened the comparison.
