Pre-charting allows you to prepare for a patient visit in advance by entering relevant information before the appointment begins. Here’s how to use this feature effectively:

  1. Open the Freed Scribe app and click on 'Capture Conversation.'

  2. Before the patient arrives, record a quick one-liner or summary about the upcoming visit and enter the patient’s name.

  3. Use the Patient Context text box to type or dictate additional details such as PMHx, labs, or other notes.

  4. Pause the visit at any time and resume recording when the patient arrives.

  5. When finished, click 'End Visit,' and Freed will process your note as usual.

Handling Patient History in Notes

Freed Scribe manages patient history in the following ways:

  • New Notes: Pre-charting information is not automatically added to new notes. Each note is generated solely from the conversation during the specific visit, regardless of whether the patient is returning or new.

  • Follow-Up Notes: Freed retains patient history from previous visits and makes it available for follow-up notes, provided the information has not been deleted. This allows follow-up notes to build on prior documentation rather than starting from scratch.

  • Manual Reference: If you need details from past encounters in a new note, you must reference or add them manually, as Freed does not automatically import details from prior visits.

Troubleshooting Note Generation

Occasionally, issues may arise that prevent Freed from generating a patient note. Common causes include:

  • The browser window being minimized or the computer going to sleep during recording, which can cause the microphone to lose access and result in no audio being captured.

To avoid this, ensure your computer remains active and the browser window stays open during the recording. Without audio, Freed cannot process and generate a note.

Ensuring Accurate Patient Identification

To prevent Freed from matching a new visit to an existing patient with the same name and pulling prior history, follow these steps:

  • Include a unique identifier in the patient’s name when creating a visit. For example: - Add a last initial: “Mike B” - Include a DOB or other unique marker: “Mike B 2015-03-02” - Mark as new: “Mike - new” or “Mike (new)”

Using a unique identifier ensures that the system recognizes the patient as new and does not auto-attach history from a different patient with the same name.
